Creating Forms

Creating forms with Reforma is intuitive and features a user-friendly interface that requires minimal training and no previous bar coding experience.

1. Scheduling & Billing - Reforma interfaces with your scheduling or billing system, creating forms on demand and pre-populated with demographic and other data. 2. Printing - Print the barcoded forms on any office printer - there's no need for outside printing or photocopying. 3. Notations - Patients sign the forms and physicians make notations, just as before – no change of behavior required. 4. Document Scanner - The annotated form is scanned using a highspeed, compact desktop scanner. 5. Online Patient Chart - Scanned forms are automatically digitized, indexed and routed to the patient’s secure online chart. The archived documents can be forwarded internally, faxed or securely e-mailed to other parties with a few clicks.
